Community Alternative Care Waiver

Organization Name: Department of Human Services
Description: The Community Alternative Care (CAC) waiver provides home and community-based services that promote the optimal health, independence, safety and integration of a person who would otherwise require the level of care provided in a hospital.

Disabilities Served: Persons who are chronically ill or medically fragile and who would otherwise require the level of care provided in a hospital.
Eligibility Criteria: 1. Eligible for the Medical Assistance program. 2. Certified disabled by Sociality Security or through the State Medical Review Team process. 3. Under age 65 at the time of opening to the waiver. 4. Certified by the primary physician to meet the level of care provided in a hospital. 5. Have an assessed need for supports and services over and above those available through the MA State Plan.
Areas served: All of Minnesota
Ages Served: Birth to 21; 22 to 65
Assistive Technology Assistance Available:
  • AT Evaluation
  • The waiver pays for the cost of the equipment, including necessary training or instruction on the use of the equipment.
