Elderly Waiver

Organization Name: Department of Human Services
Description: The Elderly Waiver is a program that funds home and community-based services for people age 65 and older who require the level of care provided in a nursing home and choose to live in the community. You must qualify for the Medical Assistance program to be eligible for Elderly Waiver services.

Disabilities Served: Persons who are age 65 or older and need a nursing facility level of care.
Eligibility Criteria: 1. Be age 65 or older. 2. Be eligible for the Medical Assistance program. 3. Need nursing home level of care as determined by the Long-Term Care Consultation process. 4. Need services that Elderly Waiver can provide for less than the cost of care in a nursing home.
Areas served: All of Minnesota
Ages Served: 66 and up

  • Funding may be used for installation, maintenance, and repair.

  • Financial Assistance
  • Direct Payments
